Can I cancel anytime?

•   Yes, you can cancel anytime, but you must give a written 90-day notice to tiffany@kimweavermd.com.  

•   In the event you wish to cancel prior to the first 90 days, you will still be charged the reoccurring payment of your                    membership.

 

Notes:

•   A consultation is required to determine the exact number of units.

•   The membership cannot be cancelled within the first three months.

•   Must use all units per quarter in one visit (no carry over).

•   Your units cannot be banked for a future quarter or future visit.

•   You can have a maximum of four visits per year.

•   Valid areas are the Glabella, Crow’s Feet and Forehead.

•   Membership is valid for 1 year after purchase.

•   Additional units will be at membership pricing.

•   Discounts apply to retail pricing and cannot be combined with other offers.
